Basic Principles of Therapy Ellen

Note:

Each part of the ear has a critical role in providing sound information to the brain. Hearing loss caused by damage to one or more parts of the outer ear, the middle or inside. In order to properly understand hearing loss, we need to understand the anatomy of the ear and how the hearing. Overview of the case will be outlined in the following explanation.

Hearing loss should always be diagnosed by a hearing, such as expert audiology or ENT specialist. They will test the hearing to determine the type and level of damage. An audiogram, will describe the findings of the hearing test.

There are four types of hearing loss:

Type the first and most common is sensorineural hearing loss caused by the loss or destruction of nerve cells (hair cells) in the cochlea. Conductive hearing loss indicates a problem in the outer or middle ear that causes terhantarnya not exactly sound to the inner ear. Normal ear anatomy
Ear consists of a narrow channel that allows the sound waves. This is called the outer ear or ear canal. This wave enters the ear canal and strike the ear drum.
Ear drum (called tympani membrane) is a membrane that vibrates as sound waves hit it. Vibrations is given to three small bones (ossicles) in the middle ear. This is called a bone hammer, stirrup bone and bone foundation.

Ossicles move to strengthen the vibration and give it to the inner ear. The inner ear contains a shell-shaped organ called the cochlea. The cochlea are tiny hair cells along the inner wall. This move in response to vibrations and send signals through the auditory nerve to the brain.

Decibel hearing

Reach normal hearing is 0-20 decibels (dB). Around 30 dB is to whisper, 50 dB for the average home and 60 dB sound for speech speeches. Sounds like a jet engine sound over 140 dB and painful.
Hearing is measured in decibels (dB HL) hearing.
25-39 dB HL hearing means light (can not hear whispers)
40-69 dB HL moderate means (not listening speech)
70-94 dB HL worse (can not hear the shouting)
more than 95 dB HL deep (can not hear the voice that will be painful for a listen)

Types of hearing loss
Types of conductive hearing loss include hearing, sensorineural hearing and the nature of the mixture.

Conductive hearing
In this case sound waves can not pass from the outer ear to the inner ear resulting in hearing. The most common reasons for:

Build-up of fluid called glue ear for ear infection
Sensorineural hearing loss

This occurs where the auditory nerve and other nerves that carry information from the brain damaged voice heard because of age or injury.
Hearing loss due to aging called presbyacusis. After the age of 30-40, many people begin to lose their hearing in small amounts. This increases with age and by 80 many people may have a significant hearing loss.

Presbuacusis occurs when the hair cells in the cochlea sensitive gradually become damaged or die. Early symptoms include loss of high-frequency noise, such as women or children voices and trouble in consonant conference, making difficult to hear and understand speech.

Ear injuries are other common causes of hearing loss. This occurs because the damage caused by loud noises. Constant inner structure was damaged due to contact with noise. Voice appears causes the hair cells in the cochlea become inflamed.

Some drugs may also cause nerve damage to sensorineural hearing ear. These include antibiotics like aminoglycosides (gentamicin, Amikacin etc.)
Mixed types of hearing loss

How can our school board access services for a child with hearing loss?



MOSD has a supraregional mandate from the Quebec Ministry of Education, Leisure and Sports (MELS) to provide services to the English School Boards for their children with hearing loss. A child who is entering a school board at age 4 or 5 is usually already diagnosed with a hearing loss, though some children may have a loss which is only diagnosed later, for example in the case of a child with a progressive hearing loss or an acquired loss due to illness or trauma.  Children suspected of having hearing loss can sometimes be tested in the audiology department at MOSD, depending on the indicators in the particular case.  Children are also tested at hospitals and private clinics. 


MOSD offers an array of services depending on the needs of the child and the distances involved.  Boards at a distance are more likely to be offered Indirect services (link).  Students attending schools closer to Montreal may receive the services of a specialist teacher of the hearing impaired as well as audiological, psychosocial and technological services through our mainstream programs ( link)

Some children with more severe losses or with more needs may benefit from attending a specialized class at the MOSD.  These classes are located within a regular public school so that partial mainstreaming may occur.  At MOSD, the goal is for children to attend their neighborhood school, as soon as they are ready and with appropriate support.  The MOSD classes serve as a bridge to the mainstream.  Academic programs link

How are services funded?

MOSD services are offered through yearly contracts with school boards. A child who had been recently diagnosed can receive services for part of a year if the board is in agreement. A child who is classified as having a hearing impairment (“44”) may require a little support or a great deal of support and specialized teaching.  Discussions between the board, the school, the family and MOSD can ensure that the appropriate program and services are made available. What are some possible signs of hearing loss in children?

I have a child in my class who may have a hearing loss. What should I do?

If you suspect that a child may have a hearing loss it is essential to pursue this. An undiagnosed hearing loss can cause a child serious problems.  Usually a general practitioner, family doctor or pediatrician will refer a child for a hearing test at a hospital or clinic. Children with many risk factors can occasionally be tested at the MOSD.

I have a child who wears hearing aids  (or a cochlear implant) in my class.  What can I do to help him in the class? 

The classroom situation is a noisy one for the child with hearing loss who needs the best acoustic environment possible in order to use his residual hearing and make use of his amplification.  Students may also use an FM system to help them hear better in noise and over distance.   FM link here.  A specialist teacher of the hearing impaired or a speech/language pathologist or audiologist can help you to determine adaptations that can be made in your classroom to improve the listening environment. I might like to volunteer at the MOSD; what could I do?

We’d love to hear from you!  There are a number of ways that a volunteer can help.

Some volunteers work in the classes or nursery with a particular group or teacher with tasks such as helping with science themes, listening to children read, helping with 1-1 tutoring, leading a lunch activity, playing games for social support. 

Some volunteers offer after-school help with homework, perhaps in French or math.  Others serve a more social role, helping the student learn more about their neighborhood, visiting the library, going to movies etc.

Other volunteers may choose to help the MOSD as an organization by working on events,  fundraising, or doing clerical work.

How are volunteers matched?


Potential volunteers come to an intake session at MOSD and are interviewed.  Their availability and preferences in relation to ages of students and types of activities are taken into consideration in making a match.

What is the time commitment?
This can be variable depending on the tasks, however in general we ask for a minimum commitment of 90 minutes a week for the school year, though we recognize that some university students may need to finish in April...

Rachmita Maun Harahap (Mita) was the fourth of six children of a couple Masniari Siregar (61) and Ali Panangaran Harahap (71). Four children their spouses (including Mita) deaf, deformed from birth. Two other normal child. Sidempuan Mita was born in Padang, West Sumatra.

Although deaf since birth, does not preclude Mita to excel in a normal school. Mita successfully passed on SDN Kertajaya 10 and SMPN 6 surabaya - then a favorite school - with a satisfactory grade. In the beginning (2001) only 20 people on Disabled, until now there are 223 on Disabled are all from the Greater Jakarta area who have participated in the program SEHJIRA. Results they achieved, including 10 people of them already know how to sew. From the sewing skill, they have opened their own business at home, and also worked at a garment company. One of their first achievement won the race to make national coat. In addition to sewing skills, 8 sudha on Disabled other works in marketing, auditing, finance, administration as well as the driver. Changing Her Fourth Limitations into Success Through Love and Prayer
Having a deaf child is a drawback for some people to be covered. But unlike the case with Masniari. February 27, 2013 | GENEVA - More than 360 million people worldwide have disabling hearing loss, according to a new global estimates of prevalence are released by WHO, for Ear Care Day International (March 3).

More people than ever face hearing loss

As the global population ages, more people than ever before to face hearing loss. One in three people over the age of 65 years - a total of 165 million people around the world - living with hearing loss. Although hearing loss from aging can often be helped with hearing devices, there is not enough produced to meet the needs.

"Current production of hearing aids meets less than 10% of global needs. In developing countries, fewer than one in 40 people who need a hearing aid have one." Said Dr. Shelly Chadha of WHO's Department of Prevention of Blindness and Deafness. "WHO is exploring technology transfer as a way to promote access to hearing aids in developing countries."
Ear infections are a major cause of hearing loss

Another 32 million are affected by hearing loss are children under the age of 15. Ear infections are the leading cause of disability, especially in the low and middle income countries. The prevalence of disabling hearing loss highest in South Asia, Asia Pacific and sub-Saharan Africa, according to the latest WHO review of available studies.

Infectious diseases such as rubella, meningitis, measles, mumps can cause hearing loss. Most diseases can be prevented through vaccination. Other common causes include exposure to excessive noise, ear or head injury, aging, genetic causes, problems during pregnancy and childbirth (such as cytomegalovirus infection or syphilis) and the use of drugs that can damage hearing.
Half of all cases are easily preventable

"About half of all cases of hearing loss are easily preventable while many can be treated through early diagnosis and appropriate intervention such as surgery implantable hearing devices. Individuals with hearing impairments may also benefit from sign language training and social support."

WHO encourages countries to develop programs to prevent hearing loss in their primary health care system, including vaccination of children against measles, meningitis, mumps and rubella, screening and treatment of syphilis in pregnant women, and the initial assessment and management of hearing loss in infants.

My Children and hearing loss
Hearing is an essential factor in children's speech and language development. Consequently, it is important that if a child has any hearing impairment, it is detected at the earliest possible stage.

It can be worrying to discover that your child has impaired hearing. For most parents it is a sorrow that has to be lived through, and it can be hard to recognise and accept that life may have turned out differently from the way we had imagined.

  • How can we communicate with our child, if he or she cannot hear? 
  • Can our child have a normal childhood? 
  • Does it hurt to wear hearing aids? 
  • Will the child be able to speak normally? 
  • Will our child have to wear hearing aids for the rest of his or her life? 
  • How will things go in school?
But do not despair. There is no reason why a child with impaired hearing should not have a good childhood and develop quite normally. Take one step at a time, and listen to the skilled, professional advice you are offered by your doctor or ear specialist.
